Hi, that particular osseous fragment looks corticated to me and so could well be the sequela of prior trauma to the ligamentum nuchae. However, looking in that vicinity, I now think there probably is subtle non-displaced fracture of the C6 posterior spinous processes (sag image 22, axial images 29).
